These could include physical therapy or wound care or extra medication management. This helps residents to maintain independence, without having to go to a hospital or nursing home for treatment. 

Here are the services typically provided by home health care:

  1. Doctor-prescribed - This is managed by the resident’s physician. The goal is to help assisted living home residents to recover from surgery, manage chronic and acute pain, or get more specialized care. 
  2. Therapeutic and Medical Services - If a resident needs a nurse, physical therapy, speech-language therapy and occupational therapy, they can get it right in their assisted living home. 
  3. Care Coordination - Home health providers work with the staff and other providers in the assisted living home to make sure everything works seamlessly to provide a comprehensive and seamless care plan for the residents. 
  4. Intermittent or Short-term Care - This is provided to residents who may need medical support that’s specialized that the regular services at an assisted living home.

Home health care also bridges the gap between an assisted living home and a hospital or nursing home. For residents, it helps the healing process much more, and for their doctors, it’s great for them to help and monitor their patients by prescribing home health care. It also helps to keep residents in their assisted living home, instead of having to relocate for care.
